ABSTRACT

OBJECTIVE: to examine the nursing team's view of the repercussions of moving patients (admissions, transfers and discharges) on the organization of work and the delivery of care. METHOD: this is a qualitative study using the focus group technique, conducted with 23 professionals - 12 nurses, eight nurse technicians and three nurse assistants working in three inpatient units at a teaching hospital in the countryside of Sao Paulo. Four meetings took place between November 2021 and March 2022. The reports were analyzed thematically using MAXQDA software. RESULTS: two thematic categories emerged: the influence of structural factors and work organization on the intra-hospital moving of patients; it demands time, generates work overload and interferes with the delivery of care. CONCLUSION: the volume of moving patient associated with unforeseen demands, care complexity and insufficient staff and resources have a negative impact on the delivery of care, with clinical risks and work overload. The findings make it possible to improve the regulation of patients entering and leaving the units, work organization and care management, avoiding clinical risks, delays, omissions and work overload. BACKGROUND: (1) Moving patients around the hospital requires structure and work organization. BACKGROUND: (2) Nursing estimates dedicating 10-15 minutes to 2-3 hours of work on these interventions. BACKGROUND: (3) Frequency, unpredictability and complexity of care have a negative impact on care. BACKGROUND: (4) Unfavorable conditions for moving generate care and occupational risks.

ABSTRACT

Background: The Children's Oncology Group (COG) is the only National Cancer Institute-supported clinical trials organization focused exclusively on childhood and adolescent cancer research. The COG Nursing Discipline Committee has embedded the tenets of evidence-based practice (EBP) into clinical trials nursing in order to standardize the nursing care delivered to children enrolled on these trials. The COG nursing EBP initiative is aimed at developing evidence-based clinical resources and tools to provide guidance to clinicians regarding topics relevant to the provision of cancer treatment for patients enrolled on COG clinical trials from diagnosis through survivorship. A rigorous, evidence-based process designed to guide development of the evidence-based clinical tools and resources within the COG nursing discipline was developed and was implemented with the first nurse expert team beginning in 2012. Method: The standardized process included (a) selecting EBP projects and nursing expert teams (NETs), (b) providing leadership, mentoring, and championship for NETs; (c) approving clinical content developed through the NETs; and (d) providing guidance and oversight over planned dissemination of the COG EBP projects. Results: The COG Nursing EBP Subcommittee has developed 15 publications to date that include 90 authors. Eleven of these authors contributed to multiple publications. Discussion: On this 10th anniversary of the development of the EBP within the COG nursing discipline, we recognize its contributions to the professional growth of many of the discipline's members and to advances in nursing care for children enrolled in pediatric cancer clinical trials.

ABSTRACT

OBJECTIVE: To identify whether safety huddle implementation enabled a change in patient safety culture. METHOD: Quasi-experimental research that assessed patient safety culture before and after safety huddle implementation. RESULTS.: The study revealed that 53.98% completed the two safety culture assessments, with 60.1% adherence from the nursing team, with a statistically significant difference in the second assessment regarding perception of patient safety and adverse events notified (p < 0.00). Regarding good practice indicators, a statistically significant difference (p < 0.00) was observed in item 43 and improvement in almost all dimensions in the second safety culture assessment. The huddles totaled 105 days, with 100% adherence from the nursing team. Regarding checklist items, all presented satisfactory responses (above 50%). CONCLUSION: Safety huddles proved to be an effective tool for communication between healthcare professionals and managers, demonstrating positive impacts on good practice indicators and most safety culture dimensions.

ABSTRACT

Behaviour towards colleagues within the nursing team that undermines teamwork and conflicts with standards and policies can be described as problematic. It is important that nurse managers understand and investigate potential problematic behaviour towards colleagues, particularly in today's healthcare environment where team cohesion and morale are constantly under threat. In this article, the author highlights the challenges for nurse managers and describes the sensitive and respectful exploratory process they need to undertake. Nurses' behaviours are underpinned by beliefs, values and attitudes so investigating potential problematic behaviour warrants an exploration of these aspects with individual nurses and with the nursing team. Therefore, the author also suggests a way for nurse managers to conduct a collective exploration of the team's values.

ABSTRACT

Abstract Objective to compare the level of dependence on nursing care and staffing between clinical and surgical inpatient units. Method This is a descriptive, retrospective study carried out in four inpatient units of a large university hospital in southern Brazil. A total of 7,486 patient classification records were included between January and October 2022. Descriptive statistical analysis, demand for nursing hours, staff projections, and the chi-square test were used. Results Intermediate care (40.2%) and semi-intensive care (40.8%) patients prevailed, with a difference between the units (p-value<0.001). There was a shortage of nurses in all the units. In one clinical unit, the projected number of nursing technicians/assistants was the same as the number available (n=46). In two units, there was a slight to moderate surplus of mid-level staff. The surgical inpatient unit with the best classification rate (92.3%) showed the greatest discrepancy between the prescribed and actual sizing. Conclusion and implications for practice the units were highly dependent on nursing care. When also considering occupancy rates and adherence to patient classification, it is plausible that one of the clinical units has the highest demand for care.

ABSTRACT

Objective: to understand the experiences of presenteeism in nursing professionals from hospital services during the COVID-19 pandemic. Method: qualitative study, anchored in historicaldialectical materialism. Thirty nursing workers participated in the research, divided into six online focus groups, analyzed based on Hermeneutics-Dialectics. Results: three categories of analysis emerged: "Worsening presenteeism in the pandemic context"; "Why did I go to work sick: the worker's decision or precarious work?"; "Old problems, permanent struggle". Despite the illness of professionals by COVID-19, presenteeism in the pandemic was marked by institutional pressure to return to work, mental suffering and lack of recognition and humanization. Among the factors that led to presenteeism, the lack of testing for COVID-19, concern for patients, co-workers and managers, as well as fear of losing their job and/or financial benefits, stood out. Faced with this scenario, workers called for a new reality in which rights such as decent wages and safe working conditions are guaranteed. Conclusion: the pandemic context revealed a worsening of presenteeism among nursing professionals. The results pointed to the importance of concretely valuing nursing in legal terms and beyond honors.

Método: estudo qualitativo, ancorado no materialismo histórico-dialético.

ABSTRACT

BACKGROUND: Quality Always' Clinical Assessment and Accreditation Scheme (CAAS), which consists of a process of assessments, reviews and accreditation in community healthcare services, was established in 2014 to drive improvements in the quality of patient care. The authors set out to understand whether the methodologies put in place to establish quality patient care were effective within the community nursing setting. AIMS: To understand whether a quality and safe care champions network would support frontline community staff to embed evidence-based practice and affect the quality of care received by patients in the community setting. METHOD: A study was undertaken on data from the CAAS, as well as patient safety and experience data, to identify whether there was any correlation between quality and safe care champions and the quality of patient care. FINDINGS: The authors identified data and practical examples to demonstrate a correlation between the engagement of champions in a community nursing team and improvement in quality standards for patients, including the most effective methods of engaging the community nursing quality and safe care champions. CONCLUSION: The authors conducted a benefits realisation exercise using elements of the East London NHS Foundation Trust's framework. This provided useful information for the community trust as a proof of concept regarding supporting a network of Quality and safe care champions in the community setting and understanding their impact on safe patient care.

ABSTRACT

OBJECTIVE: To analyze the perception of the nursing team regarding patient safety in hemodialysis clinics. METHOD: Descriptive-exploratory study with a qualitative approach, conducted with 37 professionals from the nursing team of five hemodialysis clinics, located in the cities of Sobral and Fortaleza, in Ceará, Brazil. Data were collected from October to December 2021, through semi-structured interviews recorded and analyzed by Descending Hierarchical Classification. RESULTS: Four classes emerged: operation of hemodialysis clinics; role of companions and patients in their safety in hemodialysis clinics; role of the nursing team in the safe care of patients undergoing hemodialysis; international patient safety goals in hemodialysis clinics. CONCLUSION: The participants recognize the existence of shortcomings in international patient safety goals, mainly errors in patient identification and medication administration.

ABSTRACT

OBJECTIVES: to analyze the factors that can impact patients' experience concerning safety-related measures in the hospital setting. METHODS: this qualitative, descriptive, and exploratory study was conducted with patients and their family members at a hospital in southern Brazil. Semi-structured interviews were carried out using the Critical Incident Technique between January and February 2022. The collected data underwent content analysis with the assistance of IRaMuTeQ software. RESULTS: five patients, four family members, and three patient-family units participated in the study. The following categories emerged: "Patientprofessional interaction as a component of safe care," "Recognition of safety protocols in the patient's experience," and "Safe care and the challenges in hospital care." CONCLUSIONS: patient-professional interaction, communication, awareness of safety protocols, and the availability of the nursing team are factors that influence patients' experience regarding the safety of their care during hospitalization.

ABSTRACT

OBJECTIVE: To describe the use of the Fugulin scale to classify pediatric patients hospitalized in a respiratory unit as a subsidy for the allocation of human resources given the increase in cases of Severe Acute Respiratory Syndrome. METHOD: Experience report conducted in a children's hospital in the Metropolitan Region of Curitiba with data collection from medical records and approved by the Institution and by the Research Ethics Committee. RESULTS: Between February and May 2022, the percentage of patients categorized in minimal and intermediate care decreased by 53 and 11.4%, respectively, while those in high dependency and semi-intensive care expanded by 31.2 and 84.2%. In addition, in just four months, there was a considerable increase in the positivity of virologies compared to the twelve months of 2021. The susceptibility of children to the development of severe respiratory infection was proven through the decrease in virologies with undetectable results. CONCLUSION: The results obtained allowed us to conclude there was a significant increase in the complexity of patients admitted to the respiratory unit, showing it is essential to provide a nursing team compatible with the care needs.

ABSTRACT

OBJECTIVE: To understand nursing team professionals' strategies to include the family in painful procedures performed on hospitalized children. METHOD: An exploratory-descriptive, qualitative study, carried out with nursing professionals. Data were collected through semi-structured interviews, guided by a script of topics, transcribed and submitted to thematic content analysis, in the light of Symbolic Interactionism, discussed considering the Family-Centered Care philosophy assumptions. RESULTS: Two central categories emerged, "Theoretical perspective: the family as a care agent in painful procedures" and "Practical perspective: experiences, challenges and strategies in painful procedures for family inclusion", with their respective subcategories. CONCLUSION: Nursing professionals have theoretical knowledge about family inclusion in painful procedures based on the assumptions: Family-Centered Care: dignity and respect; information sharing; joint participation; and family collaboration. However, knowledge is not applied in clinical practice; consequence of the interaction between beliefs and attitudes unfavorable to family presence.

ABSTRACT

Peritoneal dialysis is exclusively an extrarenal purification technique performed in the patient's own home. The patient is either autonomous, or assisted by a private nurse or a member of his or her family. The nursing team at the center where the patient is being cared for organizes home visits to meet the patient and his or her family in their living environment. These visits provide an opportunity to review compliance with protocols and maintain the partnership with the patient. The team at the Caen Normandy university center for kidney disease shares its experience in this area of care.

ABSTRACT

Since September 2021, at the Paris Psychiatry and Neurosciences University Hospital Group, an advanced practice nurse has been providing post-emergency consultations for people who were initially seen in the emergency department, for whom an indication for outpatient care was given and who had problems accessing care. Collaboration with the nursing team is an important element not to be neglected in the implementation of this new profession.
